Tranquil Parenting® Referral Forms
For courts, agencies, attorneys, caseworkers, counselors, educators, and other referring professionals to download, print, complete, and submit when referring a parent or caregiver for services. The forms are designed to help identify the reason for referral, areas of concern, and any court, agency, or program expectations so Tranquil Parenting can better understand the parent or caregiver’s educational needs. These forms are intended for referral and educational planning purposes and are not clinical diagnostic assessments.

1. Choose the Program
Review the four Tranquil Parenting® program areas and select the one that best matches the family’s current needs.
2. Submit the Referral Form
Complete the appropriate referral form online. Referring professionals may include relevant court, agency, case-plan, or educational expectations.
3. Individualized Review
Tranquil Parenting® reviews the referral information, family needs, child age and developmental stage, and identified concerns to determine the most appropriate educational focus.
4. Begin Instructor-Supported Learning
Participants receive individualized, trauma-informed parent education with online access to course materials for schedule flexibility and 24/7 content availability.
5. Complete the Program
Participants work through the assigned learning path, activities, and required components with instructor support.
6. Documentation of Completion
When applicable, completion verification or other requested documentation may be provided for the participant, court, agency, or referring professional.
